And then there was one document left to file. Last is the US Passport update form.

Go ahead and print the PDF document for your US Passport update form if you have not already.

Please note there are different versions of the passport update form depending on your situation.

Please look at the bottom of the form to see if it is the DS-11, S-82, or DS-5504 form.

If you are changing your gender marker, the packet should also include a medical letter. You will need to include this letter when you mail your form or go to the post office.

Please make sure your doctor has signed the medical letter before you take it in. They can sign the letter we created. Or they can use the same words and print them on paper with their logo if they prefer. If they want to create their own letter we recommend they use the same words.

DS-82 or DS-5504 form

If you are printing the DS-82 or DS-5504 form they need to be signed and dated right before the form says to continue to page 2. You will also need to write the applicants social security number in box 5 before mailing the form in.

Please note in the case of minor’s that the parent’s or guardians will need to sign as well.

DS-11 form

If you are printing the DS-11 form please do not sign it! This form needs to be signed with an agent at a passport acceptance facility.

How to pay for the update

The fees for US passports vary and depend on the form you are sending in. Please check the pages on the bottom of your form to see if you are filing the DS-11, DS-82, or DS-5504 form. The fees for your update are shown in the table below.

Filers with the DS-82 and DS-5504 forms only can mail in the form with their payment. Mailed in payments must be made by check. Please make the check out to the U.S. Department of State for the total passport fee amount.

Filers with the DS-11 form cannot mail in their form or payment. If you have the DS-11 form you do not need to write out a check!

DS-82 and DS-5504 forms only

This form will need to be mailed in. If you are changing your gender, please make sure you also include your signed medical letter.

Please include the following items with the form when you mail it in:

A certified copy of the applicant’s final name change court order
The applicant’s most recent passport (book, card, or both)
A recent color photograph 2 x 2 inches in size
A check to pay for your updates unless you qualify for form DS-5504

DS-11 forms only

If you are filing the DS-1 form you will need to go to the nearest passport acceptance facility and apply in person. The state department has a list of passport acceptance facilities here. Just enter your zip code to find the one closest to you.

When you go to the passport acceptance facility you will need to take the following items:

A certified copy of the applicant’s final name change court order
A recent color photograph 2 x 2 inches in size
Your signed medical letter (if you are changing your gender)

Passport acceptance facilities generally take either check or credit card payments. We recommend calling the facility first to make sure though.
