This form needs to be filled out to have the applicant’s name changed on their birth certificate if they were born in Wyoming. This is an important step in the process.
There is a $45 fee for having the state change a name on a birth certificate.
When you come to the end of this interview you will be able to download and print out the birth certificate update form. There are instructions on the form for paying by a personal check (third party checks are not accepted) or money order. The check or money order should be made payable to Vital Statistics Services.
This form is unique and is not filed with the Name Change Petition Packet. It also cannot be filed until the name change is final with the court. For now, keep it on the side on its own and do not add it to any other forms.
