If you are also changing the gender marker on your license you will need to fill it out.

When you come to the end of this interview you will be able to download and print out the Wyoming Driver’s License Gender Update form. You can sign and print it right away.
You also need to have a physician, therapist, counselor, or psychiatric social worker fill out and sign the bottom section of the form. This is just to have a medical provider confirm that the current gender on your driver’s license is one you no longer identify with.
This form is unique and is not filed with the Name Change Petition Packet. It also cannot be filed until the name change is final with the court. For now, keep it on the side on its own and do not add it to any other forms.
